You may be aware that NEL ICB has had a LIS/LES Equalisation programme, which was reviewing enhanced general practice services with the aim of providing a similar patient offer across NEL. Recently the ICB Executive has agreed to reshape this programme and now focus on developing a LTC Outcomes Framework for general practice with a view to commission this from 2023/24.
This framework will include CVD (covering Ischaemic Heart Disease, Stroke, TIA, Peripheral Vascular Disease, Heart Failure), Atrial Fibrillation, Hypertension, Hyperlipidaemia, Diabetes (Type 2), Chronic Kidney Disease, Chronic Obstructive Pulmonary Disease and Asthma – children and adults.
This outcomes framework will compliment and support existing services, and it will increase the opportunity for practices to receive the necessary investment to develop a similar patient offer for patients across NEL for those at risk of or living with a LTC.
We are obviously not starting from scratch as Places within NEL already have locally commissioned LTC specifications, which we are currently reviewing alongside those that exist in other ICBs. Over the next four months, we will be working closely with LTC Clinical and Commissioning Leads to develop and agree a set of outcomes and indicators for the LTC Outcomes Framework.
We will also be asking for wider input into these from clinicians and managers with skills and expertise on these subject areas. Please look out for opportunities to contribute to and improve our work.
