National pilot – proactive care at home (Long-term Conditions)

NE London, along with three other ICSs, is taking part in a national pilot to develop Proactive Care at Home for people with long-term conditions, in partnership with UCL Partners. 

Last month we asked PCNs for Expressions of Interest to take part in the pilot, and we are now pleased to share the news that 10 PCNs across NEL will be taking part in the National Pilot for people living with Long-term Conditions. These PCNs will be supported to adapt UCLPartners’ Proactive Care Frameworks, use risk stratification tools, and develop the wider primary care workforce to deliver improved models of care for people with LTCs based on empowerment, self-management and personalisation.

Discussions are taking place with PCNs in City and Hackney with a view to them rolling out the frameworks at a later date.   

This pilot provides a great opportunity to learn from the implementation process. Over the coming months these PCNs will share their learning with others across the boroughs, and we will share next steps to support the roll-out of these frameworks more broadly so that we can work towards achieving a better experience for people with long-term conditions across the whole of NE London.
