We are pleased to announce that as of the 1 December 2021 we have increased capacity for the ‘Exercise Referral Scheme’ for  individuals with Musculoskeletal (MSK) conditions. 

Patients will have the opportunity to exercise with the support of a qualified instructor, as well as receive weight management and nutrition support for a total of 12 weeks. 

Their progress will be monitored and reviewed via assessments, which will be shared with primary care providers.  

• You will need to ensure that patients referred to benefit from this program fit the eligibility criteria (attached to this communication) 
• Patients will need to contribute £12 in total for 12 weeks of sessions in order to secure their position on the sessions. This will be paid to the leisure centre they will choose to attend. 
• Upon assessment, you will need to fill in a referral form as usual (attached to this communication) for the suitable patient, which needs to be sent to: ssmith@vision-rcl.org.uk
