Additional information for GPs on referrals 

When making long Covid referrals, please make sure you are using the latest form, rather than one you may have previously downloaded, otherwise it will be rejected. 

Please also make sure you attach the completed patient form with the referral.

OneContact is a new software tool which makes it easier to complete the long COVID referral form. Patients can be sent a link to a long COVID questionnaire, which they complete using OneContact.
