The ELFT long Covid assessment service will be open to GP referrals from the 18th of January 2021 and is available to people over 18 who are residents of Tower Hamlets and Newham.
The service is an integrated community multi-disciplinary assessment service made up of professionals from a range of clinical backgrounds including physiotherapy, Occupational Therapy and psychological services. Patients will be triaged by the service and offered an assessment with the most appropriate clinician. This will be a virtual assessment in most cases and be undertaken by telephone or via an appropriate IT platform. Patients will then be referred to a range of community services for ongoing rehabilitation and monitoring.
Referrals will be accepted via email or ‘managed’ referrals in EMIS. A referral template will be available on Emis.
