Re: Discharge Medicines Service (DMS)
Over the past few months, North East London provider organisations have been working in collaboration with representatives from North Central and North East London Local Pharmaceutical Committees (LPCs), Local Medical Committees (LMC), NEL CCG, Local Primary Care Network pharmacists (PCNs) and patients to understand how best to support the implementation of the Discharge Medication Service (DMS) within Community Pharmacies.
We understand that in order for our patients to obtain the benefits of this service, secondary care organisations need to be able to effectively communicate discharge information to our Community Pharmacy colleagues.

We are extremely excited to start this journey of providing a more integrated approach to patient care and are really pleased that NEL healthcare providers have unified under this agenda as an Integrated Health System (ICS) to provide a standardised and collaborative approach to this service.
