BHRUT Anticoagulation Service


Please be reminded that referrals to the Anticoagulation service must attach a GP practice letter, which states the following:

• diagnoses
• date of recent blood tests,
• medication
• other relevant clinical information

They often get clinical information summaries attached, which are helpful for the list of medication, but nothing else to indicate the reason why the patient needs to be seen in Anticoag.

Unfortunately, without this information, referrals will have to be rejected which ultimately wastes time for the GP and has an impact on the patient.
