BHR - Phlebotomy - lack of information on bottle


We are writing to remind you that we have been advised that the laboratory at BHRUT will be rejecting samples that are inadequately labelled, the Laboratory have reported that they are still receiving a sizeable number of samples that do not have the NHS number on the sample as well as other important information. 

Please ensure that samples are appropriately labelled as per the information outlined below  to avoid samples been rejected and to minimise the impact on patients having to return to have a repeat blood test. 

Please find the 'essential' and 'desirable' patient sample and request form identification criteria, as stipulated by IBMS Criterion.

Sample:

Essential:

  1. NHS and/or hospital number
  2. Patients full name (First name and surname) or unique coded identifier
  3. Date of birth
  4. Date and time of sample

Desirable:

Nature of sample, including qualifying details, e.g. left, distal etc. especially if more than one sample per request is submitted (sometimes essential)

Request form:

  1. NHS and/or hospital number
  2. Patient’s full name (First name and surname) or unique coded identifier
  3. Date of birth
  4. Gender
  5. Patient’s location and destination for report
  6. Patient’s consultant, GP or name of requesting practitioner
  7. Date and time sample collected
  8. Investigation(s) required

Desirable:

  • Clinical information including relevant medication (which is sometimes essential)
  • Patient’s address including postcode
  • Practitioner’s contact number (bleep or extension)
  • Identified if a female is known to be pregnant (which is sometimes essential)
