Anticipatory Planning for Patients in Care Homes Prior to the Easter Holiday Period

As we approach the Easter Bank Holiday weekend, there are significant concerns around current ambulance handover times and the levels of clinical risk being incurred as a result of this.

It is particularly important at this time, that all practices with responsibility for care homes should be proactively reviewing these patients and ensuring that those at risk of deterioration over the Easter period have clear plans in place to avoid unnecessary conveyance to hospital, particularly those on an End Of Life Care pathway.
